Forms

When filling out the following forms you will notice some items are grouped in boxes, this indicates a choice between cuts.  Please choose one item from each box (unless your order is for a whole animal, then you may choose two different cuts from each box). 

When a cut has the symbol "#" next to it please write in a desired weight (i.e. 3 # rib roast).

When a cut has "in" next to it please write a desired thickness (i.e. 1 in rib steak).

If the cut has neither "#", nor "in" beside it, a check mark, or "x" is sufficient to choose that cut.  Any space left blank indicates your choice to trim and grind that item.
